Title: Frailty Syndrome: An Evidence-Based Model for Care

Activity Description/Objective: This learning activity introduces the healthcare professional to concepts related to the frail, older adult. The medical problems of the frail adult often appear overwhelming because no single disease or organ system is involved. Participants will develop knowledge, skills, and understanding of assessment, planning, delivery and evaluation of complex care management of the frail adult in any practice environment.

Target Audience: Registered Nurses

Participants will earn 3.0 contact hours of continuing nursing education.
